Basic Pitch is an audio-to-MIDI converter built by Spotify. Its web demo lets you record audio or upload a file such as WAV or MP3. The result comes back as a MIDI file that you can download and edit in a digital audio workstation. The site says it works on recordings of almost any instrument, including voice. The code is open source and linked from the site.
